In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish an End Stage Renal Dialysis (ESRD) facility in Champaign. Project #17-032, Illini Renal Dialysis, Champaign. Applicants: DaVita, Inc., and DVA Renal Healthcare, Inc. The applicants propose to discontinue an existing 12-station ESRD facility, and establish an 18-station replacement ESRD facility in 8,432 GSF of leased space, located at 1004 West Anthony Drive, Champaign. Project cost: $4,116,428.

The application contained a Safety Net Impact Statement and was declared complete on August 9, 2017. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below. Consideration by the State Board has been tentatively scheduled for the January 9, 2018 State Board Meeting.
