Notice is given you, Mark Shackelford, Father of JADE S. SHACKELFORD and to all whom it may concern: that on December 11, 2013, a petition was filed under the Juvenile Court Act by Matthew P. Banach, and that in the Courtroom of Judge Heidi N. Ladd in Courtroom C of the Champaign County Courthouse, 101 E. Main Street in Urbana, IL on February 27, 2014 at 10:45 AM or as soon thereafter as this case may be heard, a Continued hearing will be held upon the petition to have the minor(s) declared to be a ward(s) of the court and for other relief under that Act. The court has authority in this case to take from you the custody and guardianship of the minor(s).
